A formulary change question. The paperwork says one thing and the phone line says another.
One page of history, printed and handed over at the start. They read faster than you can talk.
Reconcile the medication list out loud, in the room. Silent reconciliation does not happen.
Has anyone got this through on appeal, and what did the letter actually say?
