Not a rant, an actual question about how this is supposed to work.
Get the criteria document. Every refusal is scored against a checklist and the letter never includes it.
The formulary changes on the first of January and the first you hear of it is at the counter.
Has the handling changed recently for anyone else?

Ten percent cuts, of the current dose not the original. That is the arithmetic that keeps it survivable.

Worth asking what specifically was measured. Pain scores move with mood and function does not.
