There is an error in my record and the fix seems to require a process nobody has heard of.
A clinician note carries more weight in the record than a patient request, so ask them to write it.
Portal exports often carry the numbers and not the notes, and the notes are where the reasoning is.
Has anyone got this through on appeal, and what did the letter actually say?
Access to mugs should be on the clinical questionnaire. Same for me with a wrong entry in the record, unfortunately.
Ask for the longer appointment slot. Ten minutes is not enough for a complex history and pretending otherwise wastes both your time.
Ten percent cuts. Slower is faster.
Worth asking what specifically was measured. Pain scores move with mood and function does not.
Reference ranges differ between labs, so comparing two reports is close to meaningless.
Following this because I am in the same place and have no idea what to ask at the appointment. How long have you been on a wrong entry in the record?
Function over score. Tolerance was my main issue with a wrong entry in the record too.
