Started KSM-66 (ashwagandha) six weeks ago for pain and I am trying to work out whether what I am feeling now is tolerance or something else.

You have two changes at once here. You cannot attribute anything until one of them moves.

Sorry for the basic question, but how do you actually start one of these diaries?

The pain score conversation is thirty seconds of my life I will never get back, every time.

Ten percent cuts, of the current dose not the original. That is the arithmetic that keeps it survivable. Not on ashwagandha but the pattern is familiar.

Following this because I am in the same place and have no idea what to ask at the appointment. Not on ashwagandha but the pattern is familiar.

Ask for the longer appointment slot. Ten minutes is not enough for a complex history and pretending otherwise wastes both your time. Not on ashwagandha but the pattern is familiar.

I would stop trying to convey how bad it is and start saying what you want them to do.

Access to mugs should be on the clinical questionnaire.
